Kuina's death is an unfortunate accident. It goes to show you never know what can happen in life. And another take on that is her death was an indirect result of the dual. After a katana is used it needs to be sharpened again. Which is why she went down to the storehouse to get her sharpening stone and ended up slipping and falling. So in a way if the dual didn't happen that night she may not have had the accident.
