Kushina knows, afterall she was its previous jinjuriki, which is why she had struck a deal with the demon fox. The 9 tails is to lend Naruto its power should he need it in exchange for an eternal sexual experience with her. It was difficult for Kushina at first, especially because of the size of the 9 tail's enormous dick.
