I read this as Ethan sees that Malthus is constantly hurting Al. Ethan sees by himself that Malthus is extremely abusive - before he had only Al's report and now he knows that this type of behavior is going on for long, long time.

The last scene from the sanctuary didn't show Al in a trance, and I wouldn't be surprised if that changed once Malthus upped the ante.
