I'm not sure if that amount of suffering was explained in the manga or LN for that dude, but... at least in real life it doesn't add up. A brain can go at least 3 minutes without oxygen before heavy permanent brain damage may occur, but... that doesn't mean someone's conscious all of that time. And this is when there's still a heart pumping blood throughout the body. A decapitated head is estimated to be alive for about 30 seconds at most (and probably not in the kind of state where they still have a clear mind, that would end within seconds).

He would not kill them instantly so that he would create terror/ panic across the army by having people screaming and have people get their limbs cut off. Here's an explanation of megiddo: he summons a bunch of drops of water. After that, he uses those drops of water as magnifying glasses to intensify the Sun's heat and thus creating small laser beams and he positions all of those water drops so that the beams bounce from drop to drop to a person. All of the calculations on where to place each drop is calculated by Great Sage, and since Megiddo doesn't rely on magic he can use it as much as he wants even in an Anti-magic area. Kyouva's ability makes time pass 300 times slower for him. The brain can live for around 3 minutes without oxygen. That means he was dying for 15 hours.
