Final comment I promise, in the dub, which btw I HIGHLY recommend as someone whose watched both multiple times. Irina has a really profound English saying, "Feeling are hard to kill. But they're a lot harder to resurrect." Also, Im Platinum Mad just reminded me. Another quote from the first episode is also kinda mentioned here. In the first episode, Nagisa states that "this teacher looks me in the eyes" and what Aguri said as she was dying, "If you can look them in the eyes."

There was just as much chance Irina could have been taken as the reaper, with how good of an assassin she was. It was all chance and I feel like that ways on her.

As for the reason his body changed so much is if you remember like Itona and Kaede mentioned, the tentacles asked them what they wanted to be. In Koro-sensei's case he told the tentacles, "I wanted to be weak, to be riddled with weaknesses, easy to talk to, to be able to perceive the weak, protect them, guide them, to be that kind of creature, that kind of teacher. Sometime's I'd be wrong. Sometimes my cold-hearted true face could even show. But I'll give it my all." So his appearance changed to be more approachable, not as intimidating. Sometime's his true faces would show referring to how his face changes appearance depending on his emotions and has so many weaknesses which Nagisa has recorded down since day 1.
