It's not just fighting, it's sports in general. And the gap is so big that even talentless men (who obviously do train to their limit) usually still beat the best women in a match without too much effort. That doesn't mean it's impossible for trained women to win against equally trained men, just that it's very uncommon.
Of course, anime is not real life and in anime these limits can be broken.

There is a reason that fighting tournaments of adult men and women are usually separate. In the way their bodies grow and muscles develop, men are naturally stronger than women. Woman can be beyond strong and badass but if you look at the peak of human power in both genders, there is still a fairly wide gap. Beyond society expectations, That is the wall Kuina was fighting against.

People have a theory that Kuina committed suicide, but from what I assumed actually happened is, when going downstairs to get the whetstone, she slipped with her sword in hand and she actually fell on the blade itself (instead of just breaking her neck).
