I think calling her Miko sounds better, just like you call president Miyuki, actually you call almost every character by their given names except for Hayasaka and Ishigami, which is reasonable coz their given names don't get mention a lot, their full names are Ai Hayasaka and Yu Ishigami.

I'm not sure if you guys noticed the lyrics of the school anthem but in the last part before Chika cries, it says something like "Our gratitude to [..] Our mother who raised us...". And chika is the mama :) Kashiwagi not Kawasagi Angelo. In the infirmary bed, Kaguya made a circle, it's used as a 'correct' or 'ok' sign. The opposite sign is a cross (with a buzzer for added effect) that you can sign with two crossed index fingers or by crossing your arms. They are ubiquitous symbolisms in japanese culture.
