You pieced together what was happening a lot faster than I did originally, so color me impressed. For Melba, at this point I'm reminded of a line that I adapted a bit from "The Iron Giant": We are all who we choose to be. You're right that she's definitely HEAVILY remorseful over killing Ren, and all her trauma indicates that this isn't something she has a lot of experience with or enjoys. But she still bashed his head in and folded him into that closet. All the hand-wringing in the world won't undo the actions she chose to take. All of Miller's dialogue here is made up of things that he said earlier in the series, or things that were said to him. The "bones like chalk" line is something that Anderson Dawes said to him when he was interrogating Miller back in season one, and Dawes told the story of how he killed his sister because she was sick and he had to care for the rest of his family. The "Loca Greigas" were a gang on Ceres, who were mentioned a bunch at the very beginning and then showed up as some of the hired muscle on Eros.

The thing with Miller's hat is that Holden has never seen Miller in a hat, Miller left the hat on Ceres early in season 1 before he meet Holden. So Holden is even more confused that he's seeing Miller and he's wearing a hat for some reason. As for how the Protomolecule got on the Rocinante, that's from the season 2 finale when the protomolecule hybrid got on the ship and started digging into the deck, I think at the end of the episode they showed that little glob of protomolecule residue. Also Amos and Alex were talking about their old XO going crazy is a reference to the first episode of the expanse, where the XO (second in command, XO means "executive officer") went crazy and Holden got a promotion.
