Garfiel could have stayed in that form, he turned back so he doesn't lose himself and attack everyone amongst other things. It's elaborated on more in the light novel

She never actually said her name to Emilia. I know everyone has this theory that Minerva may be her mother so I'm excited to see where this goes.

They never showed her face, only in the tea party's world/trials. Also, it wouldn't make sense for Reinhart to help because he isn't supporting Emilia and isn't her knight.
