To clear up any confusion, the scenes shown here with a normal sized Hiroshi are new scenes being added to the very start of the game. You'll be able to get introduced to the Principal before you shrink.

And for the second option you could here her heartbeat and if is slow that means she is asleep and safe to climb here throat and escape
