Yeah, you have seen Saguru Hakuba before, in Episode 219, which was another detective-gathering special, so Conan knew him already. Since the movies aren't canon, this is the first time Heiji has met Saguru. As for the weird commercial breaks, they've always been around, but since you seem to skip the openings, you probably haven't noticed them. They always give a shoutout to sponsors after the opening but before the case title. Since the opening was altered for the special, they moved the sponsor shoutouts to the door transitions. I do believe the remaining specials do the same thing, but they don't have their own dedicated window like this special. The reason why this is a special is because the first case is a "setup" case for the second case, which is the special's main event. If you really think about it, the first case does tie in with the second one. Also, I am really hoping you didn't skip the intro entirely, because the intro serves as backstory for the second case.
