A quick and easy way to create this form would be through a site called ‘JotForm’. It’s quick and easy to use and gives you access to so many templates. Here are some things you will want to list on your form:
Name
Age
Method of contact (email, social media, kik, etc)
Years of experience as a sub
References
Favorite Kinks
Hard Limits
Why they want to serve you
How much control they want you to have over them
How often they can tribute
ID Upload
You can send this form off to every sub who hops in your inbox with interest in serving you.

If anyone has issues with Jotform - you can create your own application via WIX. It was very easy

2024-01-19 12:44:35 +0000 UTCI had a jotform with all the same info that Mistress Marley shared above and my account got suspended. Has anyone else had this issue? I can't even get in touch with customer service to get it sorted out because apparently you cannot do that when you account is suspended. I didn't get a warning or notification of any kind.
