Many times I see bike lanes that are totally unusable, they are dirty and narrow causing bikes to either not use them or just ride in the lane with cars. What is the “right” way to add these types of lanes so they actually enable and protect cyclists without massive sacrifice to the cars?

As average vehicle sizes increase, could lane narrowing be an effective measure to induce demand for smaller vehicles from auto manufacturers like we continue to see in Europe?
