I was having difficulties getting Discord access via my new Patreon membership but this worked: Leave the server on Discord (if you're already in it) then go to the Patreon settings and UNLINK your Discord account and then RELINK your Discord account. This fixed the issue immediately. Hope this helps somebody out.
