Here it is, released to you, my first commission offering in a long while.
Hopefully the form will be self explanatory, but if it's full of holes, please let me know, and I'll clarify things both here and on the form itself.

[Edit: the form now properly skips he DC Character choice section if you chose an OC. Those few that submitted as of writing this edit, contact me via message or discord with any ?'s, but if you chose an OC you will only need to submit refs for your OC. -G)
