The curse that Megumi was fighting had absorbed Sukuna's finger and was cursing people for a long time but it only started killing people last June. Last June is when Sukuna manifested in Itadori. Sukuna's awakening resonated with the finger inside the curse and that is why it started activating it's curse which kills people.
