Subaru was literally almost a stranger to Otto at that point, all of a sudden while being in a life or death chase Subaru starts going crazy talking about a person who doesn't even exist and then proceeds to attack you? Most people would dump his ass of the carriage once the fact that the whale is chasing him specifically is revealed. Subaru even says that he is "exasperated by my own stupidity". The whole arc has been centered around trust imo, not knowing who is your enemy and who is your ally, not to mention the line between the two keeps getting blurred. From Garfiel and Frederica to Roswal, trust has been a major theme.

Otto telling Subaru he is a friend snapped Subaru out of his state of distrust for others, he had forgotten trust while demanding it from others at the same time.

Otto calls Subaru Yuujin in Japanese which is another word for friend (generally closer than something like tomodachi). Yuujin sounds similar to how you would say Eugene in Japanese. Edit: also, we learned earlier in the show that the witch's smell gets worse when Subaru dies repeatedly. Betty says in the mansion arc that it's so bad that she doesn't want to be in the room and then after the first Betelguese encounter (I think?) Rem also comments on it.
