Here, it's someone who lives with the vampire as a blood donor, someone they can drink from whenever they feel like, and in exchange they get lodgings and protection. It's an intimate relationship, but not equal, since one is still a food source for the other. Generally it's the most intimate a vampire will get with a human besides turning them, which is why John assumes that's what Sherlock wants, but of course Sherlock's weird and wants an equal relationship.
