From my understanding, In rear palace there are 4 high concubines (that are shown all the time). They are from powerful families. Then outside those 4. There are lot of other concubines that belong to the emperor as well. Most never even see him. Think there is example in Episode 3 of season 1. About dancer girl on the wall. That fake injurie so the Emperor doesn't have sex with her. And in return she was able to con the system to be given as prize to her childhood sweetheart.
