If you think about the causality of all these events, Alisant is somewhat responsible for the death of Helaena's son, and the heir to the throne. The two "Rats" were hired to killed Aegon by Daemon because he got information from the Worm, who was motivated to betray the Hightowers because Alisant had ordered Larys to oust all of her spies from the Red Keep.

Jeharys was the target from the get go and they tied her and alicent up amd made her choose between her 2 sons and when she refuses they threaten to rape her daughter so she picks the younger son hoping he was too young to know what is happening and then blood leans in and whispers to him see that boy ur mom wants u dead and then kills his brother like they were planning in the first place. The choice was just another way of torture bcause she never had a choice he was dead no matter who she chose.
