Ok, perhaps a bit of explaining here is necessary. As you have seen in the previous picture, Lucithia (not knowing perhaps that the Prophecy was still active) left it there on the floor and somehow what was inside of it is starting to come out to the real world. Of course Michael and Gabriel are still alive in the real world... What happened in GoC was a vision by Elokim but the contents of that very vision are now starting to get real... Does that solve your doubts? As well, it's not really known what Azariel looks like before ;)
