From my perspective, there are 4 or 5 big issues Elena has with Damon basically 1. Unlike Stefan, he loves being a vampire and loves his life the way it is, and someone like Elena isn't really on board with that. 2. Not only does Damon love being a vampire, but he doesn't avoid doing "vampire things" like feed on people and enjoy the feed and the hunt etc.. He tries not to do those things anymore but he does enjoy them. 3. She loves most of all the idea of being with Stefan and can't let go of that. Because it was a more peaceful time for her and her life was in a better place, more stable that it has been during season 3 and she wants that back. 4. I think the hardest for her is the fact that she wants to do "the right thing". That definitely means not being with Damon because literally absolutely everyone in this show does not want her to be with him(including Alaric who's his best friend, and to a certain extent even Damon doesn't really believe he can be with her). And she is afraid of people judging her, as perfectly shown in this video by Jeremy. I think the reason for that kiss in this episode was because when Damon said "I don't want to live up to anyone's expectations", that actually struck a chord with her because that is what she is always trying to do. So she just turned and sighed like she couldn't believe his mentality. And for a moment later when he says "Why not?", she just kinda let go of that fear of being judged by people. 5. She is also afraid that Damon does indeed lash out when things go wrong as proven multiple times. Anyway, this is basically my read on things. I'll come back to this list maybe in later episodes to point out why this and that make sense. Keep enjoying the show!

The chemestry between those two is off the chart. The scene is so well constructed, the sexual tention is simply spectacular. Damon statements are so epic. "Because when people see good they expect good, And I don't want to have live up to anyone's expectations". Someone made a clever connection, this sentence is mainly referred to his father, when he was trying to please him and no matter what he did he was a disappontment. So he learned to lower the bar, of what people should expect from him, not to feel anymore the pain and frustration of being misanderstood, unjustly harshly judged. This character is so deep. I loved the scene between Stefan and Alaric , but even more what Stefan told to Klaus, he can't control him anymore. Bad Alaric and Esther, what a combo, lets see what they are going to do.
