Don't know if this would be an inspiration or something, but I remembered doing a run with the restriction of toilet usage, limiting other escape routes while not letting Thomas become starved, it went rather challenging but also fun, since what's inside all turns into stamina and all you need was sheer willpower/reroll to maintain Thomas's iron bowel. (Nailed it without soiling.)
