Sly Marbo is indeed base on Sylvester "Sly" Stallone's portrayal of Rambo. While he's very good at what he does he also has the Imperium's most powerful PTSD to the point where he barely talks, and his preferred method of dealing with it is by murdering the enemies of the Imperium (which is not how you deal with PTSD). Fortunately he has an understanding commanding officer to look out for him, and makes sure Sly has all the equipment he needs to do his job. He's a lone wolf and probably one of the single most competent Guardsmen in the Imperium, completing missions that would otherwise be close to Space Marine grade. Saboutage, assassination, diversionary tactics; he's done it all. Because of this he is one of the most decorated soldiers in the Imperium, and his commander keeps all of Sly's medals in a box for him. While he was a bit of a meme before, this particular video cemented his meme status. He has a model and rules, but the model used for him here is his derpy older model. The scream itself is taken from the Dawn of War opening cinematic, just reused over and over for Sly.
