during the survey of hopeless ambitions, one of the sheets is filled out by Kouji Kumeta, who created the original Zetsubou Sensei manga. All 3 of his hopes were to win the Shogakukan manga award.

That on the clock head belongs to Kosaku Maeda - his nickname is MAEDAX, he was the voice actor for several minor parts in the show (including playing himself, credited as MAEDAX) and also worked on the manga.

Little long but you asked for it so 姓名判断 (seimei handan) is fortunetelling based on one's (full) name. It can get complicated and may even have variations of numerical interpretations depending on gender (additional implications) and whether personal names or corporation names are being evaluated. A person has 5 fortunes to be evaluated based on stroke counts (SC) of the kanji characters (also applies to kana) used in their name. 1. 天格 (tenkaku, heavenly status) - total SC of all kanji in the surname represents fortune at age 56+, ancestral/family luck - may be altered through surname change e.g. marriage (lul additional concern for the superstitious) 2. 人格 (jinkaku, personal status) - total SC of the last kanji of the surname and the first kanji of the given name (these two are adjacent) ** fortune at ages 36-55, considered the main fortune, influences personality which affects overall luck (family, profession, marriage, etc.) 3. 地格 (chikaku, earthly status) - total SC of all kanji in the given name ** fortune at ages 0-35, influences growth process (hence also personality), money, talents, sexuality, and impression from others 4. 外格 (gaikaku, outward status) - total SC of all the kanji of the full name minus the two kanji used in jinkaku ** fortune with social interactions and relationships 5. 総格 (soukaku, total status) - total SC of all the kanji of the full name general life fortune, though mainly for later years, and still heavily influenced by the previous fortunes ** For single character surnames or given names, the name with the single character may or may not be included in the calculations of jin-, chi-, and gai- kaku, depending on the consideration of a +1 "reisuu" (explained later). Other fortunes such as 仕事運 (shigoto'un, work fortune) and 家庭運 (katei'un, household fortune) are sometimes evaluated too. shigoto'un is the total SC of all kanji of the surname + the first kanji of the given name; katei'un is the opposite: total SC of the last kanji of surname + all the kanji of the given name. A stroke count number is assigned a fortune (standardized for the most part, but there can be slight variations depending on info source), which can range from very bad, bad, good, very good (sometimes even extremely bad/good). Each number also has its own specific (usually unique) fortune attribute (e.g. fame, disaster, isolation, health, authority, etc). Japanese names typically have 2 to 6 kanji in total, so stroke count numbers can range from few to dozens (one source evaluates numbers 1 to 80). In addition, there is also the concept of Yin-Yang arrangement (陰陽配列, onmyou hairetsu) for names, where a kanji is Yin if it has an even stroke count and Yang for odd. Full names with good fortune have a good balance of Yin and Yang kanji, not only regarding the amount, but also their specific positioning. (e.g. Yin-Yang-Yang or Yin-Yang-Yin-Yang are good, but Yang-Yang-Yang or Yin-Yin-Yin-Yang-Yang-Yin are bad) The Five Elements and Three Talents (五行・三才, gogyou-sansai) may also be incorporated. The three talents are 天, 人, and 地, (heaven, person, earth) which apply to the first 3 fortunes mentioned previously (tenkaku, jinkaku, chikaku). Each are assigned an element based on the "ones digit" of their stroke count. 1 or 2 is 木 (wood) 3 or 4 is 火 (fire) 5 or 6 is 土 (earth) 7 or 8 is 金 (metal) 9 or 0 is 水 (water) There are 5 generative (good), and 5 destructive (bad) dual elemental interactions (e.g. wood→fire is good since wood fuels fire, but water→fire is bad since water extinguishes fire). Fortune can be good or bad (or a mix of good/bad) depending on the 天→人→地 elemental interaction. So, for ‎糸色 望 (Itoshiki Nozomu), the stroke counts are 6, 6, 11 (even-even-odd) → Yin-Yin-Yang (considered "good" arrangement). His five statuses/fortunes are: tenkaku: 糸色 6+6 = 12 [大凶 (daikyou, very bad luck) - 挫折運 (zasetsu'un, setback/failure/frustration/discouragement fate)] jinkaku: 色望 6+11 = 17 [吉 (kichi, good) - 権威運 (ken'i'un, authority/power/influence fate)] (he's a sensei) chikaku: 望 = 11 [大吉 (daikichi, very good) - 逢春運 (houshun'un, springtime rendezvous fate)] gaikaku: 糸望 6+11 = 17 (see 17 above) soukaku: 糸色望 6+6+11 = 23 [大吉 (daikichi, very good) - 頭領運 (touryou'un, head/chief/boss fate) *note for females, 23 is associated with widow/solitude, so despite success at work, it's considered very bad luck for love/family relations] His work fortune is 糸色望 6+6+11 = 23 (very good), and household fortune is 色望 6+11 = 17 (good). The ones digits of tenkaku-jinkaku-chikaku are 2-7-1, so the Three Talents' elements are 木→金→木 (wood→metal→wood). wood→metal is not part of the 10 interactions, but for metal→wood, metal splits wood = destructive (bad). There is a catch however, as his given name, Nozomu (望) is only a single character, and so a 霊数 (reisuu, spirit number) of "+1" is added on his given name (this can also apply to single character surnames). Generally, this +1 isn't added in the full name total (soukaku). However, there are differing schools of thought on whether to specifically add or ignore this +1 in calculations for the other fortunes (jin-, chi-, and gai- kaku, when applicable). For instance, we were shown in this episode that his gaikaku remains 糸望 6+11 = 17, instead of 糸[1] 6+1 = 7 (which is good luck; independence) - but his chikaku (which includes money/sexuality) is 望[1] = 12 (the +1 is shown as [1] at the bottom), which changes it from very good (11) to very bad (12, identical to his tenkaku). So he got hit with double-zasetsu (frustration), to the point of over-influencing his other (good) fortunes. If only he could focus on the good parts. Also, without the +1 reisuu, his chikaku of 11 (springtime rendezvous) is quite nicely reflected in the sakura intro scene.
