My theory was that when he went wolf hunting with Yusef he actually shot at what he thought was a wolf but it turned out he actually shot at people who then shot back. Or that he didn't convince them he wasn't CIA so that group shot him. I was thinking the cyst on his back was actually like a bullet exit wound or something related. Then the doctor was there to operate and told him to think of a good memory (like she said when she operated on him) hence everything we saw in the movie. But he reinvented how things actually went (wanting to change things to save himself making some of the scenes feel like a dream sequence) as he died on the table. Obviously I was wrong lol!
