And given the context of zoro's backstory and Kuina being so unhappy with being a woman, this is sadly the most likely answer

And devil fruits are tricky. There are a lot of details that I won't touch on because they could be spoilers. But I will say that a certain character in a spinoff novel found a devil fruit on an uninhabited island in the East Blue when he was shipwrecked, so we canonically know that devil fruits aren't found only on the Grand Line.
