A couple of things: the woman holding the baby at the beginning is the Empress Dowager, mother of the Emperor and she seems to have given birth to him at an extremely young age. The baby she is holding is Gyokuyou's daughter, princess Lingli, also called Xiaoling by her mum (in Chinese the X is pronounced as "sh" like in "shot", so Xiaoling will read "Shaoleen" and Xiaomao - the way Gaoshun calls Maomao - is pronounced "Shaomao". Also, Maomao is not pronounced Momo but you read all the letters). Lady Lihua's baby boy died due to the poisoned lead powder.
