I think the "vascular I.D." was the movie acknowledging that in the world of Mission Impossible the masks exist, so they have to use other verification methods besides just looking at the person. Also, I have no idea why there wouldn't be automatic subtitles in the movie during the part when Ethan was strapped down. I would think that would be something permanently part of the movie print (ala when Jabba talks in Return of the Jedi), regardless if the viewer has "CC" activated or not. That was the case whenever I've watched MI3 (and I don't watch with subtitles/CC on).
