The patient begins to grow on the surgical table in front of the doctor. Maybe have a little romance because the seem not only makes him huge, but makes his testosterone levels insanely high.

The professor slips him something (chemical, pill etc) that he developed. The tiny TA becomes a certified hunk. Other idea would be the professor is the one that is small and the TA is huge. He has been researching muscle growth supplements and has created a fast acting one, deciding to test it on his favorite professor.
