Yes, Megumi was about to summon Mahoraga back then to defeat Sukuna, but it was not Itadori who took his own heart out. Sukuna had already ripped of Itadori's heart out, then he said that while he was in control of the body, he could live like that, but if Itadori took control of the body back, he would die in the spot. He was basically making Itadori a hostage to fight Megumi. Then when Megumi was about to summon Mahoraga, Itadori forced his way back to get control of the body, then he died because he had no heart. And Sukuna had 3 fingers at that point. That's also the point where Megumi used as a reference when he said that Toji was probably faster than Sukuna. Megumi only knew Sukuna's strengh of 3 fingers. 15 fingers Sukuna would mop the floor with Toji in an instant.

Just a few things: Megumi didn't need to be on the brink of death to use Mahoraga he could use it at anytime, he just can't control Mahoraga. What Gojo said was basically him inferring that Megumi has the potential to be his rival (they both have domain expansions and really strong cursed techniques), lastly the whole Sukuna needing to fight Mahoraga was that when Megumi summoned Mahoraga with Haruta (the ponytail guy) there, if Megumi had exorcised Mahoraga himself he would have been able to control him but if Haruta died the ritual would be over and Megumi would die(Megumi wasn't dead yet, Sukuna said he was in suspended animation), So Sukuna needed to save Haruta and then exorcise Mahoraga so that Megumi wouldn't die.

But the Japanese word is both a word for shrine (because it looks like a shrine) and an archaic word for kitchen (since the domain expansions use is to cut). I love little things like this so much
