The piledriver is, like any wrestling move, perfectly safe when done correctly. However, anything you do in the ring has risk, and the piledriver’s worst case scenario is instant paralysis - so for a corporation like WWE, it makes sense to have a ban for legal purposes (at least in kayfabe) On the indies up to AEW, once the bell rings it’s largely up to the workers to do whatever moves they want. Ol’ Hangman in particular likes the piledriver, fits the whole Old West executioner gimmick
