So, a little context: They show it and explain it more in the manga (Chapter 124): First off, the real Mahito acted as Blind spot so that Kugisaki wouldn't notice the switch, and due to her battle against the Double, she let her guard down against Mahito's hands. Also, she wasn't standing still down the hallway in the manga, she just slid down into the Hall after running full speed, so she didn't have time to react. Lastly, she even put up her hands to her face to try and block it with her arms. Anime made it seem like she had all the time in the world to dodge - She didn't.
