This is my interpretation of the Far Realm location, Labyrinth of Eyes from the module Phandelver & Below: The Shattered Obelisk.

This particular location represents the lair of a Beholder that has since been transformed by powerful agents into a more pathetic Oculorb. Still dangerous, it spends its hours floating about the slime-filled tunnels of its once grand lair defending it from invaders. The location has been dressed according to the module, but DM's wanting to provide some interdimensional travel for their party to fight an actual Beholder or other similar creatures can certainly use it for whatever purpose they wish!

What is the Far Realm?

The Far Realm is a plane of madness and horror situated very far from the other Planes of Existence. This maddening realm is comprised of an infinite amount of thin, translucent planes comprised of elements most mortals are incapable of comprehending. Visitors may be driven mad or twisted into gruesome monsters and it is from this realm that Aberrations came, such as Beholders, Mind Flayers, Elder Brains, Slaadi, Aboleth, Grell, Flumph, etc..

Included in the Map Pack is a Oculorb sprite, by Katsudon Art.

It comes with a token you can use for your VTT play, as well.
