Introducing... the Patreon Reward Tracker!

I've been meaning to do this for a while now, but patrons will now be able to track who has been selected for a reward for each month in the year upon completion! This will hopefully help others know when was the last time they've received a reward, but also when they'll be able to submit a form again and try for another Patreon reward.
