it only happens when a shinigami kills to intentionanlly extend the life of someone they care about. rem literally says "even if this extends your life, i will not die" when agreeing to kill L for light. if it was just that their actions caused another to live for longer, then most shinigami wouldnt exist, chaos theory and all that. its impossible to determine how removing one person from the grand equation will effect everyone else, so its not just if their actions cause someone to live longer than they should. them giving their lifespans to the perosn they samed is karma, its the cosmic way of say "ok, if you love them, then they can live, but their extra life needs to come form somewhere". we dont actually know the rules shinigami need to follow, who punishes them, the universe or the shinigami king? whats the kings actually role? if shinigmai can die, does that mean theyre born? how can they gamble with lifespans, is it a tangible thing to them? why do they exist to begin with? literally only to shorten lifespans from their natural length, but why? with all these questions, its fine that theres some confusion and ambiguity, its intentional.

That's how Shinigami survive. That is the purpose of the Death Note. However, when a Shimigami ends a human's life prematurely and their actions prevent another human's life from ending at its predetermined lifespan, essentiallysaving the human's life, they go against their nature, which is taking lives, and therefore die. And because the human's lifespan, which should have ended, has now been prevented from ending, the remaining lifespan is reset to the remaining lifespan of the Shinigami that died. Essentially, it's as if the lifespan of the Shinigami is transferred to the human whose lifespan they extended.
