As you say, significant weight loss in a normal person doesn't do that, but there's a woman on Youtube who suffers from Ehlers Danlos Syndrome (EDS), which is a connective tissue disorder that means her body lacks collagen. Her skin looks exactly the way Fat Bastard's does at the end of this movie.
