Kuvira was in the second-to-last episode of Season 3 after Korra's dad got knocked off the cliff by Zaheer and rescued by a soldier. The soldier was bandaging up Korra's dad and he thanked her and she goes "You can call me Kuvira" with the camera zooming in on her face all ominously, that's where you recognize the name from :), it was a nice foreshadowing for things-to-come in Season 4
