this is from 2 weeks ago so I doubt anyone will see it - but it's pretty common in media when otakus refer to an "object" of their affection (usually an idol, etc.) so it's pretty expressive of Subaru's character for him to immediately call Emilia in such a light, but in addition to what you said it's usually a one-sided pet name type surname, at least in the media I've seen

The only English equivalent I can think of is something like uwu speak I guess? Like saying "Mwiss Emiwiwa," although probably using -tan is less weird than using uwu speak irl.
