Regarding Helen's attitude toward Dr. Adams, I imagine she was deferring to a respected expert in her field, in much the way Kirk spoke of him early in the episode. His reputation as a successful practitioner of rehabilitative care led them to assume he was above reproach. They had no reason to suspect he was running the asylum like a mad scientist until Dr. Van Gelder was able to explain what happened to him.

While I do agree this is one of the more strange Star Trek episodes I've always liked this one. The themes and ideas explored in this episode are powerful, and this will not be the first time that Star Trek tackles the thorny subject of penal colonies and how to handle people who are in them. This was the first use of the Vulcan Mind Meld, and while it was tweaked some after this -- to become a very common thing you use in the movies and shows -- you were spot on with your feelings about it, Bunny. It's a very intimate and private experience, cause your opening your mind to someone else and the two of you 'become' the same person during the meld (hence why they were using the word 'we' when it was done in Sick Bay). They can see everything about you, and vice versa. Hence why Spock was hesitant to use it, but he decided the needs outweighed that. Also great to see McCoy sticking to his guns and going with his gut feelings here about there being a problem and forcing Kirk to take action about it. The impression I always had from this episode is that nothing sexual happened between Helen and Kirk at the party, but both of them wanted to -- she's clearly into him. But to her credit, when given the chance to exploit that she doesn't, putting her values and training first. However, due to the machine being destroyed, Kirk is stuck with those feelings for Helen and he just has to suppress them in favor of his 'first love -- the Enterprise.

I think Jim's smile at the end was his appreciation for the friends and crew that he has aboard the ship, especially of course Spock and Bones. He might sometimes feel lonely himself as the ship's captain but he'll always have them.
