It's probably just a custom skin! Basically, an overlay that changes the skin of the sim, usually found in the skin detail category. There are tons of them out there, I personally love the ones from Obscurus because they're anatomically correct and usually work well with other cc. You have to be careful when you download them though because some of them create a weird double nipple situation if you have wicked whims (which makes your sims anatomically correct in CAS)
