I know I am late to comment this but I believe the switch had to take place when Mr. Sinister kidnapped Jean after she got married to Scott in the old series. During that timeframe Sinister had plenty of time to make the clone and establish the swap. After that Sinister let Scott and Clone Jean escape with his plan in motion.

Madelyn was created when Jean was an adult because we saw in this episode Mr. Sinister creating her and they were adults. She knows she loves Scott, they just don't know when she was switched, and it only could have been around the time of the Dark Phoenix Saga. Mr. Sinister has always been obsessed with acquiring Scott and Jean's DNA because their unique genetic makeup is the perfect DNA for mutants. This was an actual story in the comics so I love how they brought this in, also Storm's storyline is going to be great!!!!

Iβll be surprised if they do anything with this love triangle though, despite it not being clear when Jean was switched, her and Scott have been in love way before sinister ever had a chance to kidnap her
