Touch late to this one, Tomo-chan ep 3 but the main diff between Rom coms and shonen/isekai is that rom coms force you to focus on characters where the other two either forcus on groups more or have too many tropes to fall back on. Rom coms live or die by their characters and have to have some meat on them.

However Tomo didn't confess her feelings for him until the first day of high school. Also, as Gundo had pointed out, Jun had avoided Tomo for nearly the entire first year of Middle school, implying that Jun had felt awkward around Tomo because he had dated her best friend despite their time together being so short. It was probably only when the animosity between Jun and Gundo had reached a level close to what It is now (likely because Jun broke up with Gundo before Gundo could break up with Jun) that Jun felt comfortable with hanging out with Tomo like he used to Loved your reaction bye the way and I can't wait until the next one.

(this is based of my personal experience not manga spoilers i havent seen past this episode) my take is that he knows shes a girl. he is definently interested in her romantically and he either doesnt know it yet or is in denial, but as director said "why" the reason is because he doesnt want their relationship to change. hes afraid that if he pursues romantic interest that everything is going to change and they wont be jun and tomo anymore. that fear is enough to allow denial to creep in and you start making excuses and push that shit down and start rationalizing and eventually it goes away for a while but that feeling keeps coming up and eventually someone is going to say someting. at some point it has to change
