The villains are have way less elite people on their side as opposed to the heroes who has everyone. Every villain also fought the opponent that counters them best. So although Nobody died but sacrifices were made, hawks lost his powers miruko has no limbs, ochaco didn’t die but it took for toga to sacrifice to survive.

One piece has a low death count too, but we’ve seen ppl get bodied/go missing so the stakes are still high since we know mfs can get bodied, same with bleach,etc.
