In the manga Inoue getting pimp slapped was brutal af. Instead of getting the saliva slapped out of her, she looked like she got half of the skin on her face removed. She was covered in blood and her arm was all fucked up. Hence the cast in the anime. Also they showed Chad's arm shredded into 3 pieces. It was detailed enough where you could see the single shred of muscle holding each piece of his arm to the other.
