As for the foster sibling, I don't mind talking about that since they never explain it in the anime. According to the novel, foster siblings share the same wet nurse. So, many times the concubines have ladies that have recently given birth share their breast milk with the the noble child. In this case, lady Ah-Duo's mother was the wet nurse for the prince (now emperor). Apparently, this causes the children to form an early bond and are sometimes as close as siblings. This also means that Ah-Duo is not from a noble family. Her mother was a wet nurse, essentially a servant. The prince (now emperor) used his influence to bring Ah-Duo to the rear palace as his only concubine. This offered Ah-Duo some protection and status. She was the prince's exclusive concubine at that time. The prince's father wasn't interested in Ah-Duo.
