The undersigned swears that he/she has reasonable grounds to believe that the above-named defendant on the 23rd day of October, 2021, at approximately 5:32 PM, at **** in Pinellas County did:

The defendant did then and there drive or be in actual physical control of a motor vehicle, to wit: a 2009 Toyota Corolla, bearing FL tag# ****, while under the influence of an alcoholic beverage and/or a chemical/controlled substance to the extent her normal faculties were impaired. The defendant was contacted after she was involved in a vehicle accident after she failed to yield while leaving a driveway. The defendant exhibited red, watery, and glassy eyes. She also exhibited slurred and mumbled speech as well as having an odor of an alcoholic beverage emitting from her breath as she spoke. She was very unsteady on her feet during the investigation. She participated in the field sobriety tests and further signs of impairment were observed.
