The reason behind 'queen' being in quotation marks is that Syracuse is not a real queen but she is extremely deranged into thinking she is. She comes from a very rich family of aristocrats which is why she can afford an estate and a full ground of servants. She has a nasty habit of kidnapping those who catch her eye for her personal collection. Many people don't complain much because it's really a life of imprisoned luxury and sex.
