I think Livia laughing at Tony as he fell down the stairs in that child-like fashion was a call-back to when Dr. Melfie asked Tony to remember a heart felt moment with his mother, and the only memory he can recall of his mother laughing was his mother laughing at his dad falling down the stairs at the Shore.
Also real quick I don't think the mob-involved family members necessarily hate each other, they're just all involved in a business where the rules dictate life or death, so when they plot to kill each other it's not at all personal (like Junior ordering the hit on Tony) but out of necessity according to the mafia's rules.

For once Tony actually does the right things (breaking up with Irina and trying to be sensitive to her mental health issues, gave his uncle a bump, helping Janice with Richie). In a way he really did save the world that day, yet it still feels like everything is wrong. He's not allowed (or incapable) of enjoying the moment. The music fits the end scene so perfectly.
Livia laughing at him was pretty painful as well. I've never seen Tony look so hurt lol
