People who receive organ transplants actually do have to take medication for the rest of their lives to prevent the body from rejecting the foreign organ. While it varies a bit, most mental hospitals can only hold you against your will for four days before going before a court and even then, only if you seem to be on the verge of causing harm to yourself or others. Talking to invisible people definitely would not warrant it. Also, I didn't really experience any volume problems in this episode even though you didn't have your ear buds.
